Telehealth Consent

This page describes the general nature of telehealth. You will be asked to review and electronically sign the controlling consent on the secure care platform before receiving care.

What telehealth involves

Telehealth uses electronic communications to exchange health information and deliver clinical services when the patient and provider are in different locations. Depending on state rules and clinical need, care may include questionnaires, secure messaging, audio, video, identity checks, and review of records or photographs.

Potential benefits and limits

Benefits can include convenient access and fewer in-person visits. Limits include incomplete information, technology failure, reduced ability to perform a physical exam, delays, privacy risks, and the possibility that in-person evaluation, laboratory work, or emergency care is needed.

Your responsibilities

You agree to provide complete and accurate information, identify all medications and conditions, participate from a private location when possible, follow provider instructions, and seek in-person or emergency care when directed. You may ask questions and withdraw consent before care, subject to services already delivered or orders already processed.

Prescribing and pharmacy fulfillment

A licensed provider independently decides whether a prescription is appropriate. A prescription is not assured. If prescribed, medication is dispensed by a pharmacy licensed for the destination state. Product and pharmacy availability may vary.

Privacy and records

Electronic systems use safeguards, but no technology eliminates every risk. Clinical records are maintained under applicable law and the privacy notices presented during intake.

Emergencies

Telehealth through this service is not for emergencies. Call 911 or go to the nearest emergency department for severe or urgent symptoms.
